(Reuters) - The move by Blue Shield, a nonprofit insurer with 3.3
million members, follows a proposal from the Centers for
Medicare & Medicaid Services in May that would limit use of the
drugs.
Blue Shield's policy change was sent out in a notice to
physicians on July 2, according to Salina Wong, director of the
organization's clinical pharmacy programs, in an interview on
Monday.
